Office Terms and Conditions

CANCELLATION POLICIES:
We kindly reserve chair time for our patients so attention can be focused on their specific needs. We ask that you please contact us 2 days before your appointment if you need to reschedule. Failure to notify us will result in a $50 cancellation fee per hour reserved. On occasion, we are asked to see emergency patients which can override our schedule. Please be aware we will notify you of any delays and be assured you will also be given the utmost attention during your appointment.
 
FEES AND PAYMENTS:
We pride ourselves in providing quality, high tech services at reasonable prices. We ask that you pay for services at the time of your appointment. We accept cash, Mastercard, and Visa. We also offer NO INTEREST financing thru Care Credit and Chase Health Financial.
 
INSURANCE:
We accept all PPO's and Delta Care HMO plans. We will bill your insurance as a courtesy. However, your plan is an agreement between you and your plan provider. Please be aware not all plans guarantee payment and any balance is your responsibility to our office. We also have trained insurance coordinators who can help maximize your benefits.
